Accident investigation: Seafarer Lost Overboard
A flag state had one of its ships report a seafarer lost overboard whilst on passage across the Atlantic Ocean, two days prior to its arrival in an EU port. The flag state had to appoint a local investigator of marine casualties to take immediate charge of the investigation and liaise with the Authorities of the port state on its arrival.
Cayman Islands Accident Investigation
On 3 November 2002 the Cayman Islands registered refrigerated cargo ship ‘ICE BIRD’ was docking in Lerwick, Shetland Islands UK. A pilot was on-board and two harbour tugs were assisting. A crew member was killed by a mooring rope being used as a towing hawser when it was placed under excessive tension due to the relative motion of the ship and the tug and rode off the top lip of the mooring bit.

IACS Audit Findings System
Through its Quality System Certification Scheme (QSCS) the International Association of Classification Societies (IACS) has responsibility for establishing and maintaining a scheme for auditing the performance of its member societies. It has a small team of auditors who travel to classification society offices worldwide and conduct audits on both offices and ships.

Specialist Consultancy to Harbour Authorities
A privately owned loss making harbour authority was in need of a complete review. We were instructed to conduct a review of all aspects of the harbour authority and report with recommendations for reversing the decline of the harbour.
ChartCo Consulting, were commissioned by a broker to attend a yacht in the Mediterranean. Our assistance was requested by a prospective buyer who wanted to purchase a Red Ensign flagged private yacht, with the view to offering it for charter when not in use by himself.
